Opening a med spa costs anywhere from $50,000 for a lean injectable-only suite to over $1 million for a premium major-metro flagship. The most common model, a standard 3 to 4 room practice, lands between $350,000 and $500,000. I have built the opening-year budget for med spas ranging from single-suite injectable startups to multi-room flagships doing $5M+ in annual revenue, and these ranges reflect what those practices actually spent, not industry averages pulled from a press release. The breakdown below covers practice model, market tier, and every line item.
Total cost by practice model
| Practice Model | Total Cost Range |
|---|---|
| Lean injectable-only or subleased suite | $50,000 to $250,000 |
| Standard 3 to 4 room med spa | $350,000 to $500,000 |
| Full-service with multiple devices | $450,000 to $700,000 |
| Premium major-metro flagship | $600,000 to $1,000,000+ |

These numbers are moving targets. Construction costs rose 4.4% year over year in 2025 (Rider Levett Bucknall), and clinical staff pay increased 4.3% (Medical Economics). Build at least a 5 to 10% buffer into every line item to account for inflation between planning and opening day.
Cost by market tier
Where you open changes the math as much as what you open.
| Market Tier | Total Startup Cost | Monthly Rent Range |
|---|---|---|
| Small town | $75,000 to $250,000 | $1,000 to $4,000/month |
| Suburban | $150,000 to $500,000 | $3,000 to $8,000/month |
| Major metro | $400,000 to $1,000,000+ | $5,000 to $25,000+/month |
A small-town practice and a major-metro flagship are not the same business, even with an identical treatment menu. Rent alone can be the difference between a $75,000 launch and a $1,000,000 one, before you have bought a single piece of equipment.
What these ranges do not include
Every range above assumes you are leasing your space and building a standard medical-aesthetics practice from scratch. A few things that can move your real number outside these ranges:
- Buying real estate instead of leasing it. That is a separate capital decision layered on top of everything here, not included in these figures.
- Acquiring an existing med spa instead of starting from zero. Acquisition often requires less upfront capital than a ground-up build, since the lease, build-out, and equipment already exist, but the purchase price itself is a different calculation entirely.
- A narrower or wider treatment menu than the standard model. An injectable-only suite with no lasers can land near the bottom of every range here. A flagship with five device platforms and a surgical suite pushes well past the top.

Once you have a real number to plan around, the next question is usually marketing. The Med Spa Launch Kit is built specifically for practices that have not opened yet: website, SEO, paid ads, CRM, and reputation management, all in place before you see your first patient. If you would rather talk it through first, schedule a Practice Growth Session. No commitment required. No credit card.
