AI Search Optimization for Med Spas: A Step-by-Step Guide

How to optimize your med spa for ChatGPT, Perplexity, and Google AI Overviews. Schema markup, llms.txt, robots.txt, entity signals, and monitoring.

A patient in your city just asked ChatGPT where to get Botox. Another one typed "best med spa near me" into Perplexity. A third one saw a Google AI Overview recommending three local practices before she scrolled to the first organic result.

Your practice was not mentioned in any of those answers.

This is not about some future shift in how people search. It is already happening. AI search platforms are generating direct recommendations with specific practice names, and in our audits across more than 50 markets, 82% of med spas did not appear in those recommendations at all.

The gap between practices that AI platforms recommend and those they ignore comes down to a specific set of technical and content signals. This guide covers exactly what those signals are and how to implement each one.

How AI search works (and why it is different from Google rankings)

Traditional SEO gets you ranked in a list of ten blue links. AI search gets you named in a direct answer. That difference changes the entire optimization approach.

Here is how each major AI search platform selects which practices to recommend:

Google AI Overviews pull from pages already ranking in Google's top 10 organic results. They synthesize information from multiple sources and present a summary with cited links. If you do not rank organically, you will not appear in an AI Overview. Strong traditional SEO is the prerequisite.

ChatGPT draws from a Bing-based index and its own training data. Domain authority carries significant weight. Content updated within the past 30 days gets cited roughly 3.2x more often than stale pages. How well your content matches ChatGPT's answer style (conversational, structured, direct) accounts for about 55% of citation likelihood.

Perplexity searches the live web in real time and always shows its sources with clickable links. It favors FAQ schema, self-contained paragraphs, and content that directly answers specific questions. It is the most transparent AI search platform and the easiest to influence quickly.

Gemini uses Google's index and Knowledge Graph. It behaves similarly to AI Overviews but with more conversational output. Your Google Business Profile data feeds directly into Gemini's local recommendations.

The common thread: AI platforms trust practices that have consistent, detailed, well-structured information across multiple authoritative sources. A polished homepage alone will not get you cited. You need depth, structure, and cross-platform consistency. The citation factors that actually matter

The Princeton GEO study (published at KDD 2024) tested nine optimization methods and measured their impact on AI visibility across platforms like Perplexity. Their findings are the most rigorous data available on what moves the needle.

Step 1: Implement schema markup on every treatment page

Schema markup is structured data embedded in your page's code that tells AI systems exactly what your practice does, where you are located, and what treatments you offer. Without it, AI has to guess this information from unstructured text. With it, you hand the AI a formatted fact sheet.

Pages with proper schema markup get cited 30 to 40% more often than pages without it.

Every treatment page on your med spa website needs these schema types in JSON-LD format:

LocalBusiness and MedicalBusiness schema. Your practice name, address, phone number, hours, geo-coordinates, and accepted payment methods. This anchors your entity identity.

Service schema. One for each treatment on that page. Include the service name, description, provider information, and price range. Link each service back to the parent MedicalBusiness entity.

FAQPage schema. Wrap your treatment page FAQ section in FAQPage schema with individual Question and Answer entities. This is one of the strongest signals for Perplexity and Google AI Overviews.

AggregateRating schema. Connect your Google review data (star rating and review count) to your business entity. AI systems use this as a quality signal when deciding whether to recommend you.

Tip

Validate your schema after implementation using Google's Rich Results Test (search.google.com/test/rich-results) and Schema.org's validator (validator.schema.org). Invalid schema is worse than no schema because it sends confusing signals to AI crawlers.

Step 2: Create an llms.txt file

The llms.txt file is a relatively new standard that gives AI crawlers a structured overview of your website. Think of it as a briefing document written specifically for language models.

Host it at yoursite.com/llms.txt. It should be plain text, not HTML.

Here is what to include for a med spa:

  • Practice name and location. Your full business name, city, state, and a one-sentence description of what you do.
  • Treatment list. Every treatment you offer, organized by category (injectables, body contouring, skin treatments, laser treatments, etc.).
  • Key pages. Direct URLs to your most important pages: treatment pages, about page, contact page, booking page.
  • Credentials and differentiators. Provider certifications, years of experience, awards, specializations. Anything that establishes authority.
  • Service area. The cities and regions you serve.

The llms.txt file gives AI platforms a clean, machine-readable summary of your practice that supplements what they find by crawling your pages. If your competitor has one and you do not, AI systems have more structured context about their practice when generating recommendations.

This file takes 30 minutes to create. It costs nothing. And it directly influences how AI models understand your practice.

Step 3: Configure robots.txt for AI crawlers

Your robots.txt file controls which bots can access your website. Many med spa websites still block AI crawlers, either intentionally from outdated advice or because they use a default robots.txt that only accounts for Googlebot.

If an AI platform's crawler cannot access your site, that platform cannot cite you. Full stop.

Here are the AI crawlers you need to allow:

Check your current robots.txt file right now. If you see Disallow directives for any of these user agents, remove them. If you do not have explicit Allow rules, add them.

Note

ChatGPT relies on a Bing-based index. If you block Bingbot in your robots.txt, you are blocking yourself from both Bing search results and ChatGPT citations. Many website platforms block Bingbot by default because site owners focus exclusively on Google.

Step 4: Optimize your entity across the web

AI models do not just read your website. They build an entity profile of your practice by cross-referencing information from dozens of sources: your website, Google Business Profile, Yelp, RealSelf, Healthgrades, social media bios, directory listings, local news mentions, and more.

When information is inconsistent across those sources, the AI's confidence in your entity drops. Low confidence means fewer recommendations.

Entity optimization means making your practice's information perfectly consistent everywhere.

NAP consistency. Your practice name, address, and phone number must be identical across every platform. Not "similar." Identical. "123 Main Street Suite 200" on your website and "123 Main St., Ste. 200" on Yelp is a discrepancy that AI models notice.

Treatment lists. The treatments listed on your website should match what appears on your Google Business Profile categories, your RealSelf profile, your Healthgrades specialties, and your social media bios. If you offer CoolSculpting, that treatment name should appear consistently everywhere.

Provider credentials. If your medical director is board-certified, that information should appear on your website, Google Business Profile, RealSelf, and Healthgrades. AI models use credential consistency as an authority signal.

Review profiles. Maintain active review profiles on Google, Yelp, and at least one medical directory (RealSelf, Healthgrades, or Vitals). AI systems treat multi-platform review presence as a signal of legitimacy. A practice with reviews only on Google raises less confidence than one with reviews across four platforms.

Your Google Business Profile is the anchor for all entity data. Get that right first, then align every other listing to match it exactly.

Step 5: Structure your content for AI extraction

AI systems extract passages, not pages. They look for self-contained blocks of information that directly answer a question. The way you structure your treatment pages and blog content determines whether AI can use it.

Answer the question in the first 40 to 60 words

When a patient asks an AI "how much does Botox cost," the AI looks for content that answers that question immediately. A page that opens with three paragraphs of marketing copy before mentioning a price range gets passed over in favor of a page that leads with: "Botox typically costs $10 to $15 per unit, with most facial treatments requiring 20 to 60 units. A standard forehead and crow's feet treatment runs $200 to $600 depending on your provider's pricing and the number of areas treated."

Put the answer first. Then expand with context, nuance, and your clinical expertise.

Use question-based headings

Structure your headings as questions that mirror how patients actually search. "How long does CoolSculpting take?" outperforms "Treatment Duration" as an H2 because AI models match query intent to heading content.

Every treatment page should have H2 and H3 headings covering:

  • How much does [treatment] cost?
  • How long does [treatment] last?
  • Am I a good candidate for [treatment]?
  • What is recovery like after [treatment]?
  • [Treatment A] vs. [Treatment B]: which is better?

Build comparison content

Comparison queries ("Botox vs. Dysport," "CoolSculpting vs. liposuction," "microneedling vs. chemical peel") account for roughly 33% of all AI citations, according to the Princeton GEO-bench dataset analysis. That is a disproportionately large share for a single content type.

For every treatment you offer where a patient might consider alternatives, publish a detailed comparison page. Include a comparison table, pros and cons for each option, candidacy differences, cost differences, and a clear recommendation based on different patient goals.

Include specific data points

"Results vary" does not get cited. "Most patients see a 60 to 80 percent improvement in skin texture within 4 to 6 weeks" does.

AI systems favor specificity: pricing ranges, session counts, downtime in days, recovery timelines, clinical study results. The more concrete numbers your content contains, the more citable it becomes.

Step 6: Build third-party citations and mentions

Your own website is not enough. AI systems place higher trust in information confirmed by external sources. Brands are 6.5x more likely to be cited via third-party sources than from their own domains.

Here is where to build your third-party citation footprint:

Medical directories. Complete and verify profiles on RealSelf, Healthgrades, Zocdoc, and Vitals. Include full treatment lists, provider credentials, and professional photos. These platforms are heavily weighted by ChatGPT and Perplexity.

Review platforms. Maintain active review generation on Google, Yelp, and RealSelf. AI models use review volume, recency, and sentiment as quality proxies. A systematic review strategy that generates 15+ new Google reviews per month with treatment-specific language signals active relevance.

Local publications. Get featured in local health or lifestyle articles. A single mention in a local newspaper's "Best of" list or a guest article on a community health blog creates a confirming data point that AI systems use to validate your entity.

Industry platforms. Respond to journalist queries through HARO, Featured, or Connectively. A quote in an industry article with your name and practice attributed creates exactly the kind of expert citation that the Princeton study found boosts AI visibility by 30%.

How to check if your practice appears in AI answers

Before you optimize, you need a baseline. Run these exact queries across all four platforms. Do not add extra context or politeness, because that changes the output.

Queries to test:

  1. "Best med spa in [your city]"
  2. "Where to get [your top treatment] in [your city]"
  3. "[Treatment] cost in [your city]"
  4. "Top rated [treatment] near [your city]"
  5. "[Treatment A] vs. [Treatment B] in [your city]"

Platforms to check:

  • ChatGPT (GPT-4o or newer)
  • Perplexity (perplexity.ai)
  • Google (with AI Overview enabled)
  • Gemini (gemini.google.com)

Document which practices appear. Note whether they are named as a direct recommendation or just mentioned in a list. If your competitors appear and you do not, study their websites for the signals covered in this guide: schema markup, content depth, review volume, entity consistency.

The relationship between traditional SEO and AI visibility

AI search optimization is not a replacement for SEO. It is a layer on top of it.

Google AI Overviews pull from the top 10 organic results. If your SEO foundation is weak, you will never appear in an AI Overview regardless of how well your schema is implemented.

ChatGPT draws from a Bing-based index that still relies on traditional ranking signals like domain authority and backlinks. Content freshness matters more for ChatGPT than for Google organic search, but the baseline authority still needs to be there.

Perplexity searches the live web, which means it uses traditional organic results as one of its input signals.

Traditional SEO and AI optimization share roughly 60% of the same work: content depth, schema markup, E-E-A-T signals, citation building. The AI-specific layer adds llms.txt implementation, comparison content formatting, entity disambiguation across platforms, and direct primary-source positioning.

Skip SEO and your AI visibility will never materialize. Skip AI optimization and you will lose an increasing share of discovery-stage patients to competitors who did not skip it.

  • Audit your robots.txt file and allow all AI crawlers listed above

Week 2: Content structure

  • Rewrite the opening paragraph of each treatment page to answer the primary patient question in the first 40 to 60 words
  • Convert generic headings to question-based H2s and H3s
  • Add 5 to 8 FAQ questions with answers to every treatment page

Week 3: Entity consistency

  • Audit NAP data across Google Business Profile, Yelp, RealSelf, Healthgrades, and all directory listings
  • Fix every inconsistency, no matter how small
  • Complete or claim profiles on any platforms where you are missing

  • Publish one comparison article for your highest-volume treatment
  • Respond to every existing review on every platform

Then run the AI visibility check again. Compare your results to your week-one baseline.
