Your med spa does not have a traffic problem. It has a visibility problem.
Patients in your city are searching for the exact treatments you offer, right now, and choosing a competitor because Google showed them first. The map pack captures the overwhelming majority of clicks on local treatment searches. If you are not in those top three positions, you might as well not exist for patients who never scroll past them.
Local SEO for med spas has shifted significantly in the past 18 months. AI Overviews now sit above the map pack for many queries. Review signals carry more weight than ever. Google Business Profile engagement metrics influence rankings in ways that did not exist two years ago.
This is the playbook that replaces our 2025 SEO checklist. It reflects what is actually working for med spa local rankings right now, not what worked 18 months ago.
What changed in local search for 2025 and 2026
Google's local algorithm updates over the past year have reshuffled the ranking factors that matter most for med spas. Here is what moved the needle.
¶AI Overviews now compete with the map pack
For treatment queries like "best Botox near me" or "CoolSculpting cost [city]," Google AI Overviews frequently appear above the local pack. These AI-generated summaries cite specific practices and pull from organic page content rather than GBP data. This means your organic treatment pages now directly influence whether patients see your name before they ever reach the map pack.
Practices with thin or generic treatment pages are invisible to AI Overviews entirely. Those with detailed, structured content that directly answers patient questions get cited. We cover this in depth in our AI search visibility guide.
¶Review signals intensified
Google increased the weight of review velocity and review content in 2025. Total review count still matters, but the algorithm now favors practices generating consistent weekly reviews over those with a high total count from years past. Reviews that mention specific treatments by name also contribute to keyword relevance for those treatment searches.
¶Engagement metrics entered the equation
Google now factors in how users interact with your GBP listing. Click-to-call rate, direction requests, website clicks, and time spent on your profile all send quality signals. A listing that gets impressions but no engagement actually drops in rankings over time. This is new, and it changes how you think about GBP optimization entirely.
¶Proximity weighting loosened for high-authority profiles
Google has always weighted proximity to the searcher heavily. In 2026, we are seeing well-optimized profiles with strong review velocity and high engagement outrank closer competitors more consistently than before. A med spa 4 miles from the searcher with 280 reviews and weekly GBP posts now beats a closer practice with a dormant profile. That was not as reliably true two years ago.
Google Business Profile: the foundation of med spa local SEO
Your GBP profile is still the single most impactful local SEO asset you own. Practices ranking in the map pack top 3 generate 5-8x more calls than those in positions 4 through 10. The gap between position 3 and position 4 is not incremental. It is a cliff.
We wrote a full GBP optimization guide that covers setup in detail. Here is what matters most for rankings in 2026.
¶Categories and attributes
Your primary category must be "Medical Spa." Not "Day Spa," not "Skin Care Clinic," not "Beauty Salon." This single setting determines which searches trigger your listing.
Add secondary categories for every treatment type you offer:
- Laser Hair Removal Service
- Botox Injection Service
- Dermal Filler Provider
- Body Contouring Service
- Chemical Peeling Service
- Microdermabrasion Service
Google caps the number of secondary categories, so prioritize by revenue contribution. Every category you add expands the search queries your listing can appear for.
Attributes matter too. Check "Identifies as women-owned" if applicable, mark your accessibility features, and set all service attributes that match your offerings. Google uses these for filtered searches and voice queries.
¶GBP posts: frequency and format
We have found that GBP posts drive 12-18% more profile views when published 2-3 times per week. Once per week is the minimum. More than three times per week shows diminishing returns.
Effective GBP post types for med spas:
- Treatment spotlights with before/after context (no graphic images)
- Seasonal treatment recommendations tied to your current promotions
- Patient education snippets that answer common questions in 2-3 sentences
- Provider introductions and credential highlights
- Limited-time offers with clear expiration dates
Each post should include a CTA button linking to the relevant treatment page on your website. This drives engagement metrics and sends users to pages optimized for conversion.
¶Q&A pre-population
Your GBP Q&A section is a ranking signal that most med spas ignore entirely. You can (and should) ask and answer your own questions. This is not against Google's guidelines. It is a recommended practice by Google.
Add 15 to 20 questions covering:
- Treatment costs and financing options
- What to expect during consultation
- Downtime and recovery for popular treatments
- Provider qualifications
- Booking and cancellation policies
Include treatment names and your city name naturally in answers. These Q&A entries are indexed by Google and contribute to your keyword relevance for local searches.
¶Photos and visual engagement
Listings with 100+ photos receive dramatically more engagement than those with fewer than 10. Upload 5 to 10 new photos monthly: treatment rooms, real results (with consent), team members, and your physical space. Geotagged photos from your actual location send an additional proximity signal.
Short videos (under 30 seconds) now display prominently on GBP listings. A 15-second walkthrough of your reception area or a provider explaining a treatment both drive engagement time on your profile.
On-page local signals that drive rankings
Your website needs to send clear geographic and topical relevance signals on every page. Here is how to structure content for maximum local search visibility.
¶Treatment pages as local landing pages
Every treatment you offer needs its own dedicated page targeting "[treatment] + [city]" as the primary keyword. A single services page listing all treatments will never rank for individual treatment terms.
Each treatment page needs:
- H1 tag: "[Treatment Name] in [City] | [Practice Name]"
- 1,000+ words of unique content covering benefits, process, recovery, pricing ranges, and candidacy
- FAQ section with 5-8 questions matching real patient queries
- Schema markup (more on this below)
- Internal links to related treatments and your booking page
- City name appearing naturally 3-5 times throughout the content
¶City and neighborhood signals
If your practice draws patients from surrounding cities or specific neighborhoods, create location-specific content that is genuinely useful, not thin doorway pages.
Effective approaches:
- A "Serving [Region]" page that explains your service area with real driving distances and landmarks
- Treatment pages that reference location-specific context ("Our Scottsdale patients often combine CoolSculpting with our...")
- Blog content addressing local topics (seasonal treatment trends in your climate, local event tie-ins)
- Testimonials tagged with the patient's city (with permission)
Do not create 15 identical pages with only the city name swapped out. Google's helpful content system identifies and penalizes this pattern. Each location mention should add genuine local relevance.
¶Schema markup for local med spas
Proper schema markup increases click-through rate from local search results by 15-25%. Google uses structured data to generate rich results, understand your entity relationships, and confirm your local relevance.
Every med spa needs these schema types implemented:
LocalBusiness (MedicalBusiness subtype)
- Name, address, phone (matching GBP exactly)
- Opening hours
- Price range
- Accepted payment methods
- Geographic coordinates
Service schema on treatment pages
- Service name, description, provider
- Area served
- Price range or starting-from price
FAQPage schema on pages with FAQ sections
- Question and answer pairs that may appear as rich results in SERPs
Review/AggregateRating schema
- Your aggregate star rating and review count (must reflect real, verifiable reviews)
Implement these as JSON-LD in the head of each page. Test with Google's Rich Results Test after implementation.
For a deeper walkthrough of schema implementation alongside other on-page factors, see our local SEO guide chapter.
Citation building: the consistency game
Citations are mentions of your practice name, address, and phone number (NAP) on external websites. They remain a confirmed local ranking factor, and inconsistencies between citations actively suppress your rankings.
¶The core citation stack for med spas
Build and verify your NAP on these platforms in priority order:
Tier 1 (highest impact, do first)
- Google Business Profile
- Yelp
- Apple Maps / Apple Business Connect
- Bing Places
- Facebook Business Page
Tier 2 (medical and industry directories)
- RealSelf
- Healthgrades
- Vitals
- WebMD Physician Directory
- ZocDoc (if you accept bookings there)
- NewBeauty
- American Med Spa Association directory
Tier 3 (general aggregators and data sources)
- Data Axle (formerly Infogroup)
- Neustar Localeze
- Foursquare
- YellowPages
- BBB
Tier 4 (local and niche)
- Local Chamber of Commerce
- City-specific directories
- State medical board listings
- Industry association directories
¶NAP consistency rules
Your practice name, address, and phone number must be character-for-character identical across every single listing. Common inconsistencies that hurt rankings:
- "Suite 200" vs. "Ste 200" vs. "Ste. 200"
- Including or omitting "LLC" or "Inc."
- Different phone numbers (tracking numbers on directories are fine only if you use the same tracking number consistently)
- Old addresses from a previous location
- Abbreviated vs. full street names ("St" vs. "Street")
Pick one format and enforce it everywhere. Audit quarterly. According to BrightLocal's Local Search Ranking Factors survey, citation consistency ranks among the top 10 factors influencing local pack rankings year over year.
¶Earning citations beyond directories
Directory submissions are table stakes. The citations that separate top-ranking practices from the pack come from:
- Local press mentions (sponsor a community event, get quoted in a local health article)
- Guest contributions to medical aesthetics publications
- Your practice featured in "best of" lists for your city
- Partnerships with complementary local businesses that link to you
- Patient stories picked up by local media
Each of these creates an unstructured citation that Google weighs heavily because it represents earned authority rather than self-submitted data.
Review strategy: velocity over volume
Reviews are now the second strongest local ranking factor behind GBP relevance signals. Your review generation strategy directly impacts your map pack position.
¶The velocity target
Aim for a minimum of 8 to 12 new Google reviews per month. Practices generating 15+ reviews monthly consistently occupy map pack positions in competitive markets. Consistency matters more than spikes. Twelve reviews per month, every month, outperforms 40 reviews one month followed by 2 the next.
¶Review content and keywords
When patients mention specific treatments in their reviews ("amazing Botox results" or "best CoolSculpting experience"), those keywords contribute to your relevance for those search terms. You cannot ask patients to include specific keywords (that violates Google's guidelines), but you can prompt them with questions that naturally lead to treatment mentions:
- "What treatment did you come in for today?"
- "How was your experience with [specific treatment]?"
These prompts in your review request flow yield reviews with natural treatment mentions without crossing into keyword manipulation.
¶Responding to reviews
Google confirms that business responses to reviews factor into local rankings. Respond to every review within 24 to 48 hours. In your responses, naturally mention the treatment type and your location. This adds keyword-rich content to your GBP without manipulation.
A strong reputation management system automates review requests, monitors new reviews, and alerts you to negative feedback requiring immediate response. This is not optional for practices serious about local rankings.
Technical local SEO that most med spas miss
These technical factors are less visible than GBP optimization or reviews, but they create the foundation that makes everything else work.
¶Mobile speed and Core Web Vitals
Over 70% of local treatment searches happen on mobile devices. Google explicitly uses page speed and Core Web Vitals (Largest Contentful Paint, Interaction to Next Paint, Cumulative Layout Shift) as ranking factors. A med spa website that loads in 5+ seconds on mobile is actively suppressing its own local rankings.
Target metrics:
- LCP under 2.5 seconds
- INP under 200 milliseconds
- CLS under 0.1
¶Local link building
Backlinks from locally relevant websites signal geographic authority to Google. Pursue:
- Local business associations and chambers of commerce
- Sponsorships of community events (with a backlink from the event page)
- Partnerships with complementary local providers (dermatologists, plastic surgeons, wellness centers)
- Local charity involvement with press coverage
- Guest posts on local lifestyle blogs or publications
One link from your city's main newspaper is worth more than 50 links from generic national directories.
¶Google Maps embedding
Embed a Google Map showing your practice location on your contact page and footer. This sends an additional proximity signal and makes it easier for Google to confirm your physical location. Use your actual GBP listing embed, not a generic pin.
Putting it together: the 90-day local SEO plan
Local SEO compounds over time. You will not jump from position 15 to position 1 in a week. But with consistent effort across all the factors above, most med spas see meaningful ranking movement within 90 days.
Days 1 through 14: Foundation audit and fixes
- Audit and correct all NAP inconsistencies across existing citations
- Set primary and secondary GBP categories correctly
- Add Q&A entries to your GBP listing
- Upload 10+ new photos to GBP
- Implement schema markup on homepage and top 3 treatment pages
Days 15 through 45: Content and citation building
- Publish or rewrite treatment pages with full local optimization (1,000+ words each)
- Submit to Tier 1 and Tier 2 citation directories
- Begin consistent GBP posting schedule (2-3x per week)
- Launch systematic review request process targeting 3+ new reviews per week
Days 46 through 90: Authority and expansion
- Build 5-10 locally relevant backlinks
- Submit to Tier 3 and Tier 4 directories
- Publish 2-4 locally relevant blog posts
- Expand schema markup to all treatment pages
- Monitor and respond to all reviews within 48 hours
Our clients average 202% increase in organic traffic within 12 months of local SEO optimization. The compound effect is real: each month of consistent work builds on the previous month. Practices that commit to this for 6 to 12 months build a local search moat that competitors cannot easily replicate.
The exclusivity advantage in local SEO
Local SEO is a zero-sum game. When you rank in the map pack, someone else does not. When your practice earns the top position for "Botox [your city]," your direct competitor drops.
This is why we work with one med spa per city. Everything we learn optimizing your local presence, every tactic that works, every competitive insight we uncover becomes your exclusive advantage rather than something we replicate for a competitor across town.
Your competitor's marketing agency is probably also working with two or three other practices in your market. Think about what that means for your local SEO strategy. The agency cannot give you an exclusive ranking advantage when they are optimizing identical signals for multiple practices targeting the same map pack positions.
Local SEO for med spas is not getting simpler. AI Overviews, review velocity requirements, engagement metrics, and entity authority are all raising the bar. But for practices willing to invest in consistent, comprehensive optimization, the reward is a patient acquisition channel that compounds every month and does not require increasing ad spend to maintain.
